Nobody wants to hear that a tooth needs to come out. But when a tooth is badly broken, infected beyond repair, or a wisdom tooth is crowding and aching, a gentle extraction is often the fastest way to stop the pain and protect the teeth around it. At The Harmony Dental Group in Norwalk, CT, Dr. Richard Malek always looks for a way to save a tooth first, and when removal really is the best option, he makes it as calm and comfortable as possible.
When does a tooth need to be removed?
Saving your natural tooth is always the first goal. Often a root canal or a dental crown can rescue it. An extraction is usually recommended when:
- A tooth is cracked below the gumline or broken beyond repair
- Deep decay or infection has damaged too much of the tooth to save it
- Advanced gum disease has loosened the tooth
- A wisdom tooth is impacted, partly erupted, or causing pain and swelling
- Teeth are crowded and need space before orthodontic treatment
If you are dealing with sudden swelling or severe pain, see our guide to emergency dental care in Norwalk. We keep time open for urgent visits.
Wisdom teeth: remove them or leave them?
Not every wisdom tooth needs to come out. If yours have come in straight, are easy to clean, and cause no trouble, they can often stay. Removal makes sense when they are impacted (stuck under the gum or bone), coming in at an angle, trapping food and bacteria, or pushing on the teeth in front of them. Our 3D imaging shows exactly where each wisdom tooth sits and how close it is to nerves and sinuses, so the decision is based on facts, not guesswork. Most wisdom teeth can be removed right here in our office, and for the rare complex case we work with a trusted oral surgeon. Our older article on when it is smart to remove wisdom teeth goes into more detail.

What to expect during an extraction
The area is completely numbed first, so you feel pressure but not pain. A simple extraction of a visible tooth often takes only a few minutes. A surgical extraction, used for impacted wisdom teeth or teeth broken at the gumline, takes a little longer and may include a few dissolvable stitches. Learn more on our tooth extractions page.
Nervous? You are in good company. We offer laughing gas and oral sedation so you can relax through the whole visit. Our sedation and comfort guide explains the options.
Recovery: the first few days
Most patients are back to normal routines within a day or two, and wisdom tooth recovery usually takes a few days longer. To heal smoothly:
- Bite on gauze for the first hour to help a clot form
- Skip straws, spitting, and smoking for at least 48 hours to avoid a dry socket
- Use cold packs for swelling and take pain relievers as directed
- Stick to soft foods and cool drinks for the first day or two
- After 24 hours, rinse gently with warm salt water
Call us if pain gets worse after day three, or if you have a fever or bleeding that will not stop.
Planning ahead: replacing a missing tooth
If the tooth that comes out is not a wisdom tooth, it is worth planning what goes in its place. A gap can let neighboring teeth drift and the jawbone shrink over time. When needed, we place a bone graft at the time of extraction to preserve the site, which keeps your options open for a dental implant later. Bridges and dentures are other good options.

Frequently asked questions about tooth extraction in Norwalk, CT
Does a tooth extraction hurt?
You should feel pressure but not pain during the procedure, because the area is fully numbed. Sedation is available if you are anxious. Some soreness for a few days afterward is normal and usually well controlled with over-the-counter pain relievers.
How long does it take to recover from wisdom teeth removal?
Most people feel much better within three to five days. Swelling usually peaks around day two and then fades. Full healing of the gum takes a couple of weeks.
Can I get a tooth pulled the same day?
Often, yes. If you are in pain, call us and we will do our best to see you quickly. Some cases, like impacted wisdom teeth, may need imaging and planning first.
What is a dry socket?
It happens when the blood clot in the socket is dislodged too early, leaving the bone exposed and painful. Avoiding straws, smoking, and hard spitting for the first couple of days greatly lowers the risk. If it happens, we can treat it quickly.
